The CHI Health Partners network, which extends throughout Nebraska and southwestern Iowa, operates in a chapter structure. Our four chapters – Omaha, Lincoln, Grand Island, and Kearney. Each chapter maintains local medical leadership and credentialing committees.

CHI Health Partners began as a physician-hospital organization named UniNet in 1998, comprised of the hospitals and aligned physicians of Alegent Health and Creighton University Medical Center in Omaha, Nebraska. Since its inception, the network has focused on bringing dedicated providers together to improve the delivery and coordination of care. This organization has been the regional leader in clinical integration and value-based care, forming the state’s first Medicaid ACO in 1999 and one of the first Medicare ACOs in the state in 2013. In 2012 UniNet received funding from CMMI through the Community-based Care Transitions Program to begin building an outpatient care management program to complement our in-patient care management team and coordinate care across the continuum. In 2012 Creighton and Alegent merge their organizations and in 2013 became wholly sponsored by CHI, which included hospitals across the state.
